Instructions:

  1. Students may declare in weeks 1-8, fall or spring semesters; no summer declaration.
  2. Please use pencil.
  3. Use transcript and course information lists to complete pages 1-3 of the form.
  4. Bring completed form, transcripts, and current enrollments to the IB Undergraduate Student Services Office.
